• Knife for Warranty repair    to #4 - Enter Value = $x.xx" (The receipt purchase value)
• Not a sale - Enter Value = NCV
• Returning to USA
• Total value - Enter same value as in #4
• Insured Value - this is your knife's Purchase value or the assesed value (#4 and #9) with the proof - rounded to hundreds)
• SDR value - if not sure - leave blank.
